Hearst Gym Fines Dispute Form

Locker Fines ($25), Uniform Fines ($40) and Towel Fines ($10) are issued in cases where our records show the respective item was not returned by the due date.
If you would like to dispute the fine and have proof that you returned the item in question, please fill out the form below.

Decisions will be issued via email about 6 weeks from the date of the appeal.

In the meantime, please pay the fine so that late charges will not accrue. If any late charges have already accrued, these charges will be removed only if the appeal is approved. If the charge is found to be valid, the late charge will also have to be paid.

To get this reference number you have to call CARS at 510-642-3190. Unfortunately, (as best as we have been able to determine) this information is not available via their web site. The number that CARS gives you will start with an "hh" like hh000001234. We cannot process your refund without this number.
